Alex and Charlie are caught in the middle of a shooting on their way to their first shift back at Hope Zion. It’s all hands on deck when the victims of the shooting end up at Hope Zee. In the midst of the commotion, Alex treats a bullied teenager and deals with her own emotions about everything that’s happened in the past year. Meanwhile, Charlie is caught off guard when he realizes that he’s still able to see the spirits of deceased patients. As well, new Chief of Surgery Joel works alongside Charlie’s ex-wife Dawn to perform an intense surgery.

He finds out that he can roam the hospital in spirit form. His fiancée and fellow surgeon, Alex Reid (Erica Durance), tries to save his life with the help of other doctors, including surgeon Joel Goran (Daniel Gillies), as they struggle with their day to day lives to keep the hope alive at Hope Zion......Charlie Harris (Michael Shanks), the charismatic Chief of Surgery of Toronto’s Hope-Zion Hospital ends up in a coma, leaving the hospital in chaos – and his fiancée and fellow surgeon, Alex Reid (Erica Durance), in a state of shock. Along with newly-arrived star surgeon, Joel Goran (Daniel Gillies), Alex races to save Harris' life. As the action unfolds, comatose Dr. Harris explores the hospital halls in “spirit” form, not sure if he's a ghost or a figment of his own imagination.
